When you provide personal information in order to enroll and maintain your insurance coverage, we consider that an act of trust. We have safeguards in place to ensure that all personal information is protected and kept confidential. We never sell information about you to others and reveal only what is necessary in the course of completing transactions related to you and your coverage.
And while information is sometimes exchanged with third parties, as standard in our industry and permissible by law, it is done so with your protection in mind.
In compliance with the Gramm-Leach-Bliley Act of 1999 (GLBA) and the Health Insurance Portability and Accountability Act of 1996 (HIPAA), we are providing you with this document, which notifies you of the privacy policies and practices of Rogers & Norman, Inc.

Information we collect
We collect personal information about you, your family and your employees from the following sources:
- 1. Information you provide on applications or other forms (such as your name, address, financial information and driver's license number).
- 2. Information we receive from your medical records or medical professionals.
- 3. Information about your transactions with us, non-affiliated third parties or others (such as your payment history and claims information).
- 4. Information we receive from a consumer-reporting agency (such as your credit history and motor vehicle report).
Information we receive from credit reports or investigative consumer reports
Before completing an insurance transaction, Rogers & Norman, Inc. may be required to obtain information about you from credit reports
or other investigative consumer reports prepared by third parties. If you authorize us to request such information, you should be
aware that:
You have the right to request to be interviewed in connection with the preparation of such a report.
Upon request, you are entitled to receive a copy of the report. The information obtained from the report prepared by the third party may be retained by the third party and disclosed to other persons.
Persons from whom information is collected
Rogers & Norman, Inc. does not collect nonpublic personal information from anyone other than the individual(s) proposed for
coverage.
How we use your personal information
The information you share with Rogers & Norman, Inc. is used for completing insurance and health related transactions that you
request. We do not disclose any nonpublic personal information, or protected health information, about our customers or former
customers to anyone, except as permitted by law.
For example, we may share collected information to our non-affiliated service providers when necessary to carry out our normal business activities. These activities may include underwriting requests for insurance products, administering policies, administering claims and processing transactions at your request. Health Care operations may include: premium rating, or other activities relating to the creation, renewal or replacement of another contract of health insurance, ceding, securing, or placing a contract for reinsurance or risk relating to claims for health care. Non-affiliated service providers may include the insurance carrier, financing company, leasing company and/or Mortgage Company. In addition to those circumstances, and unless you tell us not to, we may also disclose nonpublic personal information about you to third parties that perform marketing services on our behalf or with whom we have joint marketing agreements. Our marketing services deal with fulfillment service providers, such as envelope stuffing services.
Personal health information
Rogers & Norman, Inc. obtains medical information only in connection with specific insurance products or claims. In addition to
medical history, this information may include details regarding an accident, disability or injury. We will not use or share
personally identifiable medical information for any purpose other than to complete an insurance transaction or for the administration
of your policy, claim or account.
How we protect your information
Our employees are trained and required to maintain our privacy policies and procedures. Employees who violate these policies and
procedures are subject to disciplinary action. Non-affiliated third parties with which we disclose information are required to
maintain adequate security standards for the protection of collected information. Rogers & Norman, Inc. maintains physical,
electronic, and procedural safeguards to protect information about you.
Changes in our privacy policy
Rogers & Norman, Inc. may choose to modify this policy at any time. We will notify customers of any modification on an annual
basis.
What are your rights?
You have the right to request access to the personal information that we record about you. Your right includes the right to view such
information and copy it in person, or request that a copy of it be sent to you by mail (for which we may charge you a reasonable
fee). You also have the right to request corrections, amendments or deletions of any information in our possession. If you believe
that your rights with respect to our protection of your health information have been violated, you may contact the Secretary of
Health and Human Services or submit a detailed complaint directly to us. Please submit any request in writing to Rogers & Norman,
